Initial Med Evaluation — $300 without insurance/private pay rate
Comprehensive psychiatric and medication evaluation, health history, collaborate on goals and treatment plan - approximately 60-75 minutes.
Extended Med Follow up or Therapy only with NP -$175 without insurance/private pay rate
Medication management follow up for existing clients with medication concerns plus the use of supported counseling or option of therapy only services. Approximately 45-50 minutes.
Our therapists have a sliding scale between $150-$100/session
Basic Med Follow Up - $150 without insurance/private pay rate
Medication evaluation and adjustment, if appropriate, for current clients. Review of symptoms, goals and treatment plan along with supportive counseling. Approximately 20-30 minutes
Coaching $175
Feeling stuck and like you aren’t moving forward with your goals? If you are not needing therapy to process emotions/feelings/trauma, but looking coaching instead, this is also a service that is offered and can be offered to clients outside of CA, OR or AZ.
Late Cancellation/No Show Fee is $150. If you don't show or call, please be aware that you will be charged a standard fee for the missed appointment. You must cancel/reschedule with at least 24 hours notice, so we can attempt to contact others on the waiting list to fill the spot.
If there is an urgent situation or unexpected medical or personal emergency that you needed to cancel your appointment - please don't hesitate to contact us by phone or by email, so we can work together to find a workable solution.
*Refill request outside of appointments is $50. Please pay close attention to when you are running low or out of medication and do your best to schedule an appointment BEFORE you run out of medication. Please make sure to allow at least 5-7 days at minimum before you run out of medications. We do not routinely refill your medications outside of appointments. If there is a pattern of refill requests without appointments, this private practice may not be the best fit for your treatment, and we would be happy to provide you with referrals to colleagues working in larger group practices or local clinics, who tend to have additional on-call services and the benefit of multiple providers.
*Forms and paperwork, phone consults - outside of appointments are billed based on the standard hourly rate of $300. This is reserved for extraordinary clinical circumstances such as complex case coordination, comprehensive clinical and legal forms, and/or extensive phone consultations outside of appointments, exceeding 10 minutes per week. It is best to address forms, paperwork and consults together during a scheduled appointment.
There is no charge for brief and routine telephone calls for administrative tasks such as scheduling appointments or for billing questions.
Payments & Insurance
Currently we take Optum (UnitedHealthcare, Sutter, Western Health Advantage) and Aetna insurance and we are able to provide private pay services for those with a PPO or without insurance.
Superbilling is provided per client request for reimbursement from your insurance (please contact your insurance directly for reimbursement request). If your plan offers "out-of-network" coverage, we do not bill the insurance directly for you. We provide you with a “superbill” payment invoice, upon payment. You are then able to submit this receipt to your insurance company for direct reimbursement.
Co-payment is due at the time of service. If there is an unpaid balance, we will ask you to provide payment during your visit to cover this. This includes any unpaid visit balances.
If you are more than 30 days past due with a visit balance, we will do my best to reach you to address payment before any future appointments are scheduled. If you do not contact us after several attempts, your balance may be 1) turned over to a collections service and/or 2) need to terminate our treatment relationship due to non-payment.
